Insurance Effective Date: The date the patient got insurance

Insurance Term Date: This should be left blank unless the patient’s insurance has actually termed. Please do NOT term a Medicare policy if the patient switches over to cash. Leave the term date blank, open a Cash account, and link the two accounts. Please only term an insurance if the patient has legitimately ended one insurance and opened another.

Policy Effective Date: If the policy started at the first of the year, please list January 1 as the Policy Effective Date. This should not be listed as the first date the patient came to the clinic, unless the patient’s Policy happened to be effective the first date he/she came to the clinic.

Policy Term Date: This should be left blank unless the Policy itself has actually terminated.

Policy Group Name: If the policy has no group name, please leave this field blank. This is not a required field.
